I'm with Media Cybernetics, a scientific image processing/analysis software company. Our Image-Pro Insight software has tools that allow you to easily draw freeform lines, annotations, ROIs and alter the size, width, color, font size/type, etc.  

If working with timelapse images, you can create annotations that appear on specific frames of your video - to point out something that just happened. You have the option to save as .AVI to embed the annotation within the video (to share with others) - or - you can save your timelapse image and the annotation will appear the next time you open the video file (within the software).

you can have a look at endrov,
www.endrov.net
which places a lot of emphasis on working with large files, and on annotation. I don't know if all the annotation support you need is in there yet but if not, let us know how we can improve it
