Hi,

I'm trying to get my hands on one of the older spinning disk publications that first described the CSU microlens design:

High-speed confocal fluorescence microscopy using a Nipkow scanner with microlenses for 3-D imaging of single fluorescence molecule in real time.
A.Ichihara, T.Tanaami, K.Isozaki, Y.Sugiyama, Y.Kosugi, K.Mikuriya, M.Abe and I.Uemura
Bioimages 4, 57-62(1996)

I couldn't find a copy of the article you're looking for, but this one
came up and looks OK:
