"double-vision" through oculars

I am having an issue with my Olympus IX81 microscope.  We are seeing "double" through the oculars.  Both the interpupillary distance and dioptric parameters for each ocular have been adjusted correctly -  but we still are seeing "double".

Anyone have any ideas/recommendations???

Misalignment of the ocular splitter to the microscope axis can occur. Can be seen sometimes when the ocular head is rotated by 180. Can you turn the head which holds the eyepieces upside down? This may fix the problem -also check that the ocular head is properly seated.

This happened to us as well. There is a prism that we needed to have swapped out in the eyepiece. It cracked somehow. Its a defect that Olympus is aware of. You may want to call your Olympus sales rep to have it fixed unless youre under contract.
